To request a copy of your medical records, you must do the following:

• Complete an Authorization Form for Release of Information.
• Provide a valid form of picture identification at the time of your request.
• The fee for records is a routine rate in accordance with Illinois Statues & Regulations; you may further contact Release of Information,
  at (815)936-3206  for a copy of the most current fee schedule. 
•  Average processing time for record requests is 5 - 7 business days from the date of request. This time may vary as per the volume of records requested.
