Provena Home Care

Guiding Principle: Diversity

Living in a rural area can be relaxing and quiet, allowing individuals to live a peaceful, restful lifestyle. Unfortunately, it can be burdensome to have to travel far distances to receive quality medical care and adequate emotional and physical support. Through specialized services, Provena Home Care is able to reach out to those in remote areas.

Dorothy Forbes, a Provena Home Health patient from Grant Park, IL, awoke one morning in extreme pain, unable to move from her bed. Unfortunately, her medical alert device encountered a glitch and didn’t immediately connect her to help. When Joe Toth, physical therapist with Provena Home Health, arrived for a scheduled appointment, he found Forbes still in extreme pain. Toth called her doctor, who told him to call an ambulance. Since they were in a rural area, while they waited, Toth took a physical assessment of Forbes, carried her to her living room and helped her dress. He then checked her medical alert device to make sure it was back in working order.

Forbes was touched by Toth’s compassionate care. So much so that when another Provena Home Health employee, Lisa Vandervliet, RN, had a nursing visit with Forbes, she shared how Toth’s expertise and compassion saved the day.

“She was crying the whole time she told me the story of how Joe helped her,” Vandervliet says. “She truly felt that everyone from Provena really cared about their patients.”

EASING TRANSITIONS

Provena Hospice’s Transitions program, based in Champaign, is another example of Provena Home Care’s commitment to complete patient care, with special attention paid to those who live remotely. The program offers assistance and support to individuals coping with a life-limiting illness, roughly half of whom live in rural areas. 

Linda Tauber, Transitions Coordinator for Provena Home Care and Hospice, explains, “Many times chronically ill or cancer patients are unable to work and have little other support and often become overwhelmed by their illness. Transitions provides referrals to address financial concerns, to assist with transportation to medical appointments, to help with housekeeping and running errands. Transitions also provides much needed emotional and spiritual support to help patients focus on taking care of themselves.”

Through the dedicated care and compassionate support of Provena Home Care and Provena Hospice, patients who might not otherwise have access to high quality services are able to receive care in the comfort of their own home by skilled clinicians. This is especially valuable for rural patients, who would have to travel many miles to receive the same care in a clinical setting, and the time and stress of doing so could compromise their course of treatment. Luckily, Provena Home Care and Hospice is able to travel the miles for them.
