Medical and Vital Records

You may obtain your medical record information from our Health Information Services Departments at each ministry. Everyone MUST complete an AUTHORIZATION TO USE AND DISCLOSE HEALTH INFORMATION FORM.

There may be fees assessed for generating copies of medical records.  Costs are generally determined on a per-page basis.  Additionally, charges to copy microfilm will also be quoted.

Most requests are routinely completed in 5-7 business days.
