PMG Benefits Program

Provena Health offers a flexible benefits prorgram for contracted Provena Medical Group physicians. 

Those physicians eligible to receive benefits include those working full-time (at least 35 hours per week, or 70 hours per pay period) and part-time (at least 20 hours per week, or 40 hours per pay period).  You can choose coverage for your eligible dependends as well, including your legal spouse and your unmarried children up to age 19 (or up to age 25 if they are full-time students).  Children include natural children, stepchildren, legally adopted children or children for who adoption proceedings have begun, and children under your legal guardianship.

Your coverage under the Provena Health flexible benefits program begins on the first day of employment.  Your dependents are covered under the plans when you are, provided you have enrolled them.

Provena Health offers four different coverage levels to meet your specific needs: individual coverage (for you only), employee plus child(ren) coverage (for you and your child or children), employee plus spouse coverage (for you and your spouse) and family coverage (for you, your spouse and your child or children).

Medical Plans

Provena Health offers two medical plan choices in most areas - a Health Maintenance Organization/Exclusive Provider Organization (HMO/EPO) and a Preferred Provider Organization (PPO).

Prescription Benefit

Dental Plans

Provena Health provides dental benefits by offering two options.  The dental options are offered through MetLife - a Traditional Plan and a managed plan called the Preferred Dentist Program (PDP).

Vision Plan

The Vision Service Plan (VSP) provides you the opportunity for significant savings on annual vision examinations and corrective lenses.

  • Professional Services: VPS covers one exam per year
  • Materials: VSP covers one pair of lenses (prescription glasses or contacts) per year and one pair of frames every 24 months.
  • Scheduled payment: the amount VPS reimburses a participant when a non-network provider is used

Life Insurance

Provena Health provides Life and Accidental Death & Dismemberment (AD&D) insurance.

  • Full-time employees: two times annual salary rounded to the next higher $1,000 (minimum of $10,000; maximum or $250,000)
  • Part-time employees: one time annual salary rounded to the next higher $1,000 (minimum of $10,000; maximum of $250,000)

In addition to the Core Life and AD&D insurance, eligible full-time and part-time employees can also purchase Supplemental Life Insurance.  Eligible employees also have the option to purchase dependent life insurance.

Eligible dependents include:

  • Your spouse, up to age 70 if you are covered under the Core Life Insurance Plan
  • Your unmarried, dependent children who are between 14 days and age 19 (or age 25 if they are full-time students)

Continuing Medical Education

As per physician contract, physicians are entitled to a specific number of paid education days per calendar year to attend medical conferences and continuing medical education sessions.  Additionally, physicians are provided annual professional education reimbursement.

Paid Time Off

Provena Health offers an annual vacation program.  As per physician contract, physicians are entitled to a specific number of vacation days per calendar year.  Any unused vacation days will not carry over to the next calendar year or be paid out.

Disability Coverage

For extended illnesses, salary continuation will be utilized.  Long-Term Disability (LTD) insurance (coverage of 50% of annual salary) is available to all benefit eligible full-time employees.  A buy-up option (to 66 2/3% of salary) is also available.  LTD may begin after 90 days of continued illness.

Provena Health offers spending accounts and provides you with two options to help you save on tax payments.  You may elect to participate in either, both or none of the options offered.  Your options include:

  • Dependent Care Spending Account

Retirement Program

The Provena Health Retirement Program is a defined 403(b) contribution plan.  Consisting of a base contribution, voluntary employee salary deferral and an employer matching contribution.
