 With over 15 years experience in care nursing working with chronic disease at Presence Covenant Medical Center, Teresa Westenhaver, Advanced Practice Nurse with the Presence Center for Healthy Aging, is a part of the Health and Wellness Clinic team that provides the following services:
- Primary care with an emphasis in chronic disease management
- Welcome to Medicare Visits and Annual Medicare Wellness Exams
- Preventative care such as immunizations and cardiovascular and diabetes screenings
- Comprehensive assessment that includes spiritual, emotional, cognitive abilities and physical well-being

- Patient-centered care with longer appointment times, case coordination, case management, and monitoring and follow-up
- Health insurance and prescription assistance counseling, transportation assistance, completion of advanced directives and health education classes at no additional cost to patient
