Helping You Heal at Home
There's just something comforting about home that makes it an optimal place for care and recovery. So when your physician recommends it, we bring our special brand of caring right to your front door. From short-term home care assistance to the special work of our hospice unit, we bring the care you need to the place you love the most...home.

What You Need. Where You Need It
Presence Covenant's Home Care nurses work together with families to coordinate the care of patients living at home. Nurses teach them how to care for specific needs and provide skilled nursing care and treatment as ordered by the patient's physician. Home health aides are also available to help patients with simple therapy, bathing, shampooing and other procedures, under the direction of a nurse or therapist.When patients need help adjusting at home, medical social workers are available to help resolve problems. These specially trained counselors help patients and their families cope with stress, develop long-range plans and utilize available community resources.For patients recovering from illness and injury, therapists are available to maximize patient recovery. When treatment is prescribed by a physician, physical, occupational and speech therapists are also available to provide care and instruction at home.

A Special Brand of Caring
The Presence Covenant Hospice Care Program provides comfort and support for patients who are terminally ill. With compassion for their patients and strength for the families who help them, Presence Covenant Hospice nurses deliver a special brand of caring to homes in Champaign-Urbana and the surrounding communities.

The Team Approach
The Hospice team creates an individualized care plan for each patient they assist. The team consists of physicians, nurses, home hospice aides, social workers, dietitians, chaplains, pharmacists, volunteers and bereavement counselors.

Grief Support
Hospice offers grief support groups to help survivors cope with the death of a loved one. Support groups provide educational and emotional support for children and teens, as well as older adults.
