Provena Home Care Joins National Campaign to Reduce Rehospitalizations

Provena Home Care has signed on as one of the first agencies to participate in the Home Health Quality Improvement National Campaign which seeks to reduce avoidable hospitalizations and save Medicare dollars.  This initiative falls very much in line with our commitment to service excellence and to continually improving the quality and safety of patient care. 

The campaign unites groups such as the National Association for Home Care and Hospice, American Association for Homecare, The Illinois Home Care Council, CMS and its contractors, which includes the Illinois Foundation for Quality Health Care as a Medicare Quality Improvement Organization (QIO), under a shared vision of reducing avoidable hospitalizations to improve patient quality of care.

Participating with the other organizations involved in this campaign will enable us to promote best practices, give our patients every opportunity to safely remain in their home environment, and optimize clinical performance, while monitoring data driven outcomes.
