Adult Day Center Services


The perfect place for your loved one!  We know that you want to be there for your loved one, but there are times when responsibilities make that difficult.  Designed for adults 60 and older, our Adult Day Center offers caregivers and families the added support they need during the weekday hours.

Presence Heritage Day Break has been serving the community for over 25 years.  We provide care, handicap accessible transportation, support and socialization for older adults in a compassionate, safe and stimulating environment. 

Hours of Operation 

 The Adult Day Center serves clients Monday through Friday from 7:00 a.m. to 5:30 p.m.  Learn more about the daily activities going on at our center!

Payment Options 

 Whether your loved one is above or below 60 years old, we have found that financial assistance is available for most of the clients that we serve.  In addition, private pay rates are available.  We encourage you to call our office at (815) 937-2447 to learn more about the financial assistance options available to you.

Applying for Adult Day Services

There are three ways that you can apply for adult day services.

  • For private pay clients, we simply ask that you come to our ministry for a tour - or take a virtual tour - and complete an admissions application.
  • Clients who are age 60 and older may be eligible for funding through Catholic Charities (our case coordination unit), and in order to determine eligibility, we refer potential clients to this organization who then completes an assessment of various needs, including financial, physical and mental needs.  This assessment determines if an individual is eligible to receive financial assistance.  If Catholic Charities does determine he/she is eligible, they refer the individual back to our adult day center to complete the admissions form.  We begin providing services to the client as soon as we receive the contract.
  • We refer clients who are under age 60 and have a disability of some type to the Illinois Department of Rehabilitation who also provide a need-based assessment.  If individuals are eligible, the Illinois Department of Rehabilitation will refer them back to our adult day center to complete the admissions form, and we provide services as soon as we receive the contract. 

Transportation

Presence Heritage Day Break offers reliable, daily transportation in the Kankakee and surrounding area for our regular clients.  Our courteous, professional drivers will pick up your loved one in our trustworthy, handicap accessible vehicles at a designated time.  And, safely transport them home at the end of the day.
