Community Living Facility

At Provena Life Connections, our team of highly trained professionals provides 24-hour supervision to adults with developmental disabilities in a personalized, home-like setting.  Residents at our community living facility have deficits in at least three of the six life areas: self-care, language, learning, self-direction, mobility, capacity for independent living, and self-sufficiency. 

We offer a regular schedule of community and on-site social opportunities.  Helping the residents to maintain their personal dignity and independence are our primary goals.
