My Medical Record
To help you better manage your healthcare, the MyChart Medical Record module allows you to review your medical history, including your health summary, medications, allergies and immunization history.
MyChart also allows you to see test results electronically. The results are released to your physician practice first and then to you. For your convenience, MyChart will send you an e-mail to alert you when your test results are available. Additional information about normal values for that particular test and other test details are also available. Of course, if the lab results are sensitive, your physician will still contact you directly to discuss in further detail.
You can also view a list of procedures or immunizations recommended for you based on age, gender, and other medical factors. Due dates help you decide when to request tests or procedures.

Message Center
The Message Center allows you to exchange messages with your Provena Medical Group care team. MyChart provides an inbox that keeps a record of all messages you have sent and received. MyChart communications are secure and may become part of your medical record. Of course, if your issue is urgent, please call your medical provider or seek immediate care. For billing questions, it is recommended you call your Provena Medical Group provider.
The "Get Medical Advice" feature allows you to securely send non-urgent questions to your Provena Medical Group healthcare team, and gives patients two options that both involve secure messaging and follow best practices in security and HIPAA compliance:
Ask a General Medical Question
Patients can ask general medical questions and can even attach pictures to the message that can become part of their medical records. MyChart's messaging system is not an emergency response system and is not to be used as a substitute for calling 9-1-1 or for seeking immediate medical attention at your local emergency room in the event of an emergency. Provena Medical Group providers and staff will make their best efforts to respond to messages submitted via MyChart within two (2) business days; however, timely responses are not always possible. Accordingly, urgent and/or emergent messages should not be submitted via MyChart.
Begin an E-visit
E-visits allow you to communicate with physicians about non-urgent health concerns. E-visits include questionnaires designed to help physicians diagnose patients. Patients choose a symptom, such as sinus pain or cough, and then answer a series of questions specific to the symptom. The physician then reviews the questionnaire responses and the patient's chart to determine a diagnosis, order medications or lab tests, and enter a charge for services without the patient ever coming into the office. If further follow-up is required, the physician can send a message asking the patient to come in for a standard office visit. If you choose to engage in an E-visit with members of the Provena Medical Group care team, it is your responsibility to check with your applicable insurance plan about coverage for E-visits and any applicable cost sharing amount for which you may be responsible.
Also in the Message Center...
You can even renew your prescriptions through the Message Center! Requests are reviewed and approved by your Provena Medical Group provider, which then forwards the prescription order to your pharmacy. Patients can also use the "Referral Request" feature to send secure messages to their physicians to obtain referrals to specialists.

Appointments
With 24-hour online access to MyChart, simply log on at any time to request or cancel an appointment with your doctor. It's that easy! Within the Appointment Center, patients can view history from past appointments, review details about upcoming appointments (including location and driving directions) or request to schedule an appointment with a Provena Medical Group provider. The office will review the appointment request and contact you with any questions.

Family Records
The Family Records section allows you to manage health records for your family members. This includes parents or legal guardians of minor children (up to age 12) as well as those adult patients not under guardianship and otherwise capable of making their own healthcare decisions who wish to allow another adult proxy access to their MyChart account. You will need to fill out a specialized "proxy" form located online in our Terms and Conditions section or at your Provena Medical Group physician's office in order to access another person's medical record information. Completion of release forms and verification are required for this access. Please note that the HIPPA authorization form for adult proxy access has a term limit of two years, upon which time it will need to be completed again for continued acccess.

The MyChart Health ID Card provides a convenient wallet-sized printable summary of your health information, including your contact information; medical conditions, medications and allergies; provider contact information and insurance details.

Linked Healthcare Records
Provena Medical Group wants to be your primary healthcare provider, but if you're seen at another organization and also use their MyChart patient portal, you can create a central account to help you quickly and easily manage your personal health information from multiple sources.
- MyChartCentral: combines MyChart accounts from multiple healthcare organizations so that patients have a single Web site to access all his or her MyChart accounts without needing to remember multiple passwords.
- Lucy: allows patients to create permanent copies of their charts from organizations they've linked through MyChartCentral. Patients can use Lucy to store and share their own health information or health summaries from all of their healthcare organizations. Within Lucy's "My Personal Health Records" section, patients can add personal information about their allergies, medications, health concerns and other information. As a result, the patient's Medical Document Library contains both patient-entered data and documents from the patient's healthcare organizations.
