Provena United Samaritans Medical Center believes in healthy living, which is why we provide you with the opportunity to learn about your health, and potential problems, often before you have any symptoms.

HealthAware is free, confidential, and offers different health modules that take only about 7 minutes to complete and there is no obligation. After completing your evaluation, you will be able to print out a custom report.
